Filling In a Personal-Details Form

Hook

Hotels, schools, gyms, doctors, websites: almost everywhere asks you to fill in a form. Forms use short, special words, and a mistake can cause real problems (a wrong date of birth, a missing email). Let’s make sure you can fill in any simple form confidently.

  • Title: Mr / Mrs / Ms
  • First name: Maria · Surname / Last name: Pérez
  • Date of birth (DD/MM/YYYY): 12/05/1998
  • Nationality: Spanish
  • Address: 14 Park Road, Leeds, LS2 7EF
  • Phone number: 07700 900123 · Email: [email protected]
  • Signature: M. Pérez · Date: 28/09/2026

Are there any full sentences on the form?

Rule

  • Forms need short answers, not sentences: First name: Maria (not My first name is Maria).
  • First name = your own name · Surname / last name / family name = your family’s name.
  • Date of birth = your birthday with the year. UK: DD/MM/YYYY (12/05/1998 = 12 May). US: MM/DD/YYYY (05/12/1998 = May 12). Check the format!
  • Nationality = Spanish (not Spain). Country = Spain.
  • Address: number + street, town/city, postcode.
  • Title: Mr (man) · Mrs (married woman) · Ms (any woman) · Dr
  • Use CAPITAL LETTERS if the form says “Please write in block capitals”.
  • Signature = how you sign your name. Occupation = your job. Marital status = single / married.

Watch Out

  • ✗ Nationality: Spain → ✓ Nationality: Spanish
  • ✗ First name: My name is Ali. → ✓ First name: Ali (short answers only)
  • ✗ Surname: Ali (when Ali is your first name) → ✓ Surname: your family name
  • ✗ 05/12 = 5 December (on a US form) → ✓ on a US form 05/12 = May 12 (check the date format)
